项目摘要

本研究の目的はビブリオ・バルニフィカス(以下Vibrio vulnificus)に対する迅速な診断法を確立し、本症の救命率を向上させることにある。平成20年度にV.vulnificus感染を疑った5症例に対し患者家族から同意を得て、患者背景、臨床症状、既往歴等の情報収集を行うとともに、提供を受けた創部の滲出液や血液検体について簡易迅速培養検査および遺伝子増幅検査を行った。その結果、5例中2例において遺伝子増幅検査(LAMP法)では検査開始から約5時間、2種類の培地による簡易迅速培養検査では約1日で患者収容医療機関に情報提供を行うことができた。また、この結果は、検査開始から2日後に得られた従来の培養結果と一致した。残念ながら、本感染症の確定診断に至った2症例ともに医療機関受信時すでにショック状態を呈していたため、早期診断結果も救命には繋がらなかった。感染モデル作製報告はないため、経口感染による実験動物モデルの作製に着手した。今回は肝機能が正常なマウスを用いて感染実験を実施したが、今後の臨床的研究推進のためには肝障害マウスの作製が必要となる。従って現在下記手順に則り肝障害マウスの作製と感染実験を計画中である。
