本研究の目的は「浣腸による直腸穿孔を防ぐために肛門管の形態を検索する」ことにより,「浣腸による直腸穿孔を予防する」ための基礎的資料を得ることである。この目的に従い,肛門管粘膜上皮の形態を光学顕微鏡・走査電子顕微鏡を用いて検索した。肛門と肛門管は重層扁平上皮からなり,肛門管直腸移行部(anorectal junction)から上部の直腸は単層円柱上皮からなっていた。肛門管は歯状線によって上部(肛門柱と肛門洞)と下部(肛門櫛)に二分され,上皮の様相も異なっていた。歯状線下部の上皮は15～30層で軽度に角化しており,一方歯状線上部は30～40層の非角化重層扁平上皮であった。重層扁平上皮は保護作用を有することが知られており,肛門管は物理的刺激に対して比較的強い構造を有していることが明らかとなった。また,歯状線下部はわずかに角化していることから歯状線下部のほうがより強い保護作用を有していることが推察される。今回の所見のうちでもっとも注目すべき点は,肛門管の重層扁平上皮から肛門管直腸接合部を境界とし,急激に単層円柱上皮に移行することである。このような形態的様相は噴門(食道から胃)に類似しており,肛門管直腸接合部の上部と下部とではその機能が大きく異なることが推察される。重層扁平上皮は保護作用を有しているが,肛門管直腸接合部より上部の単層円柱上皮は保護作用が少なく,粘液の分泌,吸収作用が中心である。肛門縁より肛門管直腸接合部までの長さは3.5-4.5cmであり,それよりも上部(直腸側)では急激にカテーテル挿入などの物理的刺激に対して弱い構造へ移行していることが今回の結果で明らかとなった。さらに直腸,肛門管の粘膜固有層には多数の毛細血管を認め,走査電子顕微鏡観察において肛門管の重層扁平上皮の最表面と直腸の単層円柱上皮が同じ高さの平面状にあった。肛門管直腸接合部よりも上部(直腸側)の粘膜上皮はカテーテル挿入などの物理的刺激により損傷を受けやすいというだけでなく,粘膜上皮が損傷した場合出血しやすい形態をしていることも示された。

The purpose of this study is to obtain the basic data on the prevention of rectal perforation. For this purpose, the morphology of the anal mucosa epithelium was examined by optical microscopy and electron microscopy. The anal canal is divided into layers of squamous epithelium, and the anal junction is divided into upper layers of columnar epithelium. The anal canal is divided into two parts: the upper part (anal column and anal hole) and the lower part (anal comb), and the epithelium is different. The lower part of the dentate line has 15~30 layers of keratinized epithelium, and the upper part of the square dentate line has 30~40 layers of non-keratinized stratified squamous epithelium. The stratified squamous epithelium has a protective effect, and the anal canal has a protective effect against physical stimulation. The lower part of the tooth line is cornerized and the lower part of the tooth line is protected. This time, we can see that the stratified squamous epithelium of anal canal is at the junction of anal canal and rectum, and the stratified columnar epithelium is at the transition point. The upper part of the anal canal and the lower part of the rectal junction are similar to each other. The stratified squamous epithelium has a protective effect. The stratified columnar epithelium at the upper part of the anal junction has a protective effect. The secretion of mucus and absorption are central. The anal canal is 3.5-4.5cm long at the anal junction, and the upper part of the anal canal (rectal side) is stimulated by physical stimulation. In the rectum, the mucosa proper layer of the anal duct is the most common layer of capillaries. The electron microscope examination shows that the most surface of the stratified squamous epithelium of the anal duct and the stratified columnar epithelium of the rectum are the same. The mucosal epithelium of the upper part of the anal junction (rectal side) is damaged by physical stimulation, and the mucosal epithelium is damaged by bleeding.
