项目摘要

近年、化学療法・放射線療法の進歩により小児固形腫瘍の治癒率は80%に向上している。治癒後の成長発育の過程において、これらの治療によって生じた形成異常によるQOL (Quality Of Life)の低下を主訴とする症例も増加している。そのため、QOLの改善を図ることが現在の小児歯科医療に強く求められていることを日々の臨床を通じて痛感している。治癒率の向上によって新たに生じてきた問題もあり、医科ではすでに長期にわたるフォローアップ体制の必要性が示唆されている。しかしながら、長期フォローアップ体制も設立されたばかりで、まだ小児悪性腫瘍を経験した患児に対する長期的な歯科的管理のEBMの確立がないため、歯科は枠組みに組み込まれていない。そこでEBMの確立を目的として、当科管理中の小児悪性腫瘍患児に対して、(1) 小児悪性腫瘍の現状、(2) 歯科受診状況、(3) 口腔内の状況、(4) 治療によって惹起された、もしくは惹起されると考えられる歯科的異常所見、(5) 患児あるいは保護者が口腔内のことに関して不安に感じていること、(6) 患児あるいは保護者が日常生活で不都合に感じていること、(7) 患児あるいは保護者が日常生活における口腔内ケアで気をつけていること、以上の7つの項目に関するデータ収集を行った。
