本研究の目的は,頭頸部がんで喉頭全摘出術後患者(以下,患者)の日常生活に支障をきたしている内容の分析結果から,術後患者が社会生活へ適応できるための看護モデル構築を検討することである。前年度までに患者は失声と同様に「食べること」にも支障をきたしていることが明らかとなったため,今年度は患者の社会生活への適応を阻害する要因を「食べること」に絞り,患者の食生活の実態を明らかにする。患者の食生活を「食の意義」「食行動」「健康管理」の3側面から捉え,術前期,術後期,放射線・化学療法期,治療終了期における患者の食生活に対する主観と客観の縦断的調査を行い,その分析結果から患者の食生活を改善するための指導指針を提示し,活用することを目的としている。調査項目を精選するため,頭頸部がん患者の食生活の問題点および食の意義が含まれる国内外の文献を検討し,患者の食生活の要素の分析までが終了した。頭頸部がん患者の抱える食の問題点として,味覚障害,唾液分泌の減少,摂取時の疼痛,食欲の減少,悪心・嘔吐,下痢・便秘,嚥下・摂食機能の低下,ボディーイメージの変化,摂取時に周囲の目が気になる,気分の落ち込み,食生活の楽しみがない,食後の疲労感等が抽出された。食の意義については,健康,食欲,嗜好,楽しみ,交流,活力,安全などが抽出された。これらの結果を用いて,平成23年4月中に調査を実施し,平成23年度中に結果を出す予定である。

The investigation items were selected, and the problems and significance of food and life of patients with head and neck diseases were discussed. The analysis of the factors of food and life of patients was completed. The head and neck patients have food problems, taste disorders, decreased saliva secretion, pain at the time of eating, decreased appetite, nausea, vomiting, diarrhea, constipation, decreased swallowing function, changes in food quality,"time of eating","fatigue after eating", etc. The meaning of food, health, appetite, hobbies, happiness, communication, vitality, safety, etc. The survey was conducted in April 2013, and the results are expected in the middle of 2013.
