项目摘要

【研究の具体的内容】(1)文献研究:ホスピス及び緩和ケア領域における市民参加に関する先行研究を収集し、分析した。(2)調査研究:地域緩和ケアチームを運営する2ヶ所の診療所で参与観察を行った。専門職種と一般市民が協働して緩和ケアチームを運営するための、マネジメント方法、条件、方策を検討した。【意義】在宅での緩和ケア体制が整備されていない地域では、がん難民をはじめとする社会的弱者が生まれる危険性が高まる。特に緩和ケア期の患者は、在宅療養の開始に伴い社会との繋がりが希薄になりやすいことから、社会的孤立に陥る可能姓が高まる。この際、専門職からの支援だけではなく、患者と同じ地域で暮らし、地域特性や慣習を共有しながら患者と対等に向き合える一般市民のサポートが有用となると考える。本研究では、これまで十分に活用されてこなかった地域の資源に焦点を当て、専門家の持つ専門知と一般市民の持つ一般知を融合させた、地域ぐるみの在宅緩和ケア体制を提案する。研究の成果は、患者の生活の質の向上に寄与するだけでなく、従来の在宅緩和ケアモデルに新たなインプリケーションを提案するものであり、医療、看護、福祉全般に対して意義のある研究になると考える。【重要性】(1)新しい人材として一般市民の参加に着目したことにより、医療的ニーズだけでなく、これまで見落とされていた社会的ニーズへの対応が可能となる。WHOがめざすHolistic Approachを実現する在宅緩和ケア体制が提案できる研究となる。(2)市民参加型の在宅緩和ケア体制は、地域内で有効に活用されていない高齢者、主婦、退職者といった人的資源の活用を視野に入れたものである。専門職では対応しきれない領域をアウトソースすることにより、医療・福祉双方で高いコストパフォーマンスが期待できる。マンパワー不足の解消や医療費の抑制といった政策課題にも対応し得る成果が期待される。
