项目摘要

平成22年度は,平成21年度において明らかとなった抑うつの強い子どもの注意・記憶バイアスの特徴である「ポジティブな情報を回避する注意バイアス」とその脆弱性としての機能の臨床的応用について検討を行うことを目的に以下の2つの研究を実施した。第一に,「ポジティブな情報を回避する注意バイアス」が抑うつに与える影響とそのプロセスを縦断的研究によって検討した結果,注意バイアスは対人関係上のストレッサーを介して抑うつを強める際に,「周囲から認められた自分の経験」の評価や「周囲が自分を助けてくれるという信念」を低下させていることが示された。すなわち,こうした注意バイアスは,子どもが友人関係などの悩みを強く抱えている際に,その悩みを低減させるような過去の経験を思い出すことや考え方を阻害することで,抑うつを強めるという継時的なプロセスが明らかとなった。続いて,上記の知見に基づき,「ポジティブな情報を回避する注意バイアス」の変容を目的とした全3回の心理教育・認知訓練から成る予防介入プログラムを開発し,地域の小中学校3学級にて実践を行った。介入効果の検討を目的に,質問紙法による抑うつや先行研究にて抑うつとの関連が示されている自覚可能な認知行動的要因の測定、および実験法による注意・記憶バイアスの測定を介入2週間前,介入1週間後,介入1ヵ月後の3時点で実施した。その結果,開発された介入プログラムによって注意バイアスの変容が可能であること,また注意バイアスの変容が抑うつの長期的な予防に寄与することが示された。これらの結果を通じ,注意機能という自覚困難な認知プロセスにおけるネガティブな歪みであっても,実験という体験的な手法を通じて検出・変容可能であること,その変容が抑うつに対する長期的な介入効果を有することが示唆された。
