项目摘要

当研究室では本邦地域在住高齢者の「生活の質」(Quality of Life : QOL)や「豊かな老い」(Optimalageing)の実態を探るため、日本国内3町村フィールド(北海道浦臼町、三重県大台町、高知県土佐町)での包括的高齢者機能調査(Comprehensive Geriatric Assessment : CGA)を1990年代から行っている。平成25年度は、上記3町の65歳以上の地域在住高齢者(施設入所、入院しておらず、家で暮らす高齢者)約4000人分のデータを用い(2006年分)、自記式質問紙である転倒スコア(21項目)と健康指標(歩行、食事、入浴するなどの身体的な日常生活機能、物忘れなどの認知機能、抑うつなど)、主観的健康観(self-rated health : 自分をどのくらい健康だと思うか)の関連について、どのような傾向があるのか解析した。その結果、主観的健康観と転倒スコアは、年齢やうつ傾向などの項目で調整しても、互いに有意に関連していることがわかり、今後の大規模なセルフチェック型検診において、転倒スコアが主観的健康観も反映する可能性が示唆された。主観的健康観の調査方法では、自分が自分をどのくらい健康と考えるかをダイレクトに尋ねる手法を取るが、主観的な尺度が転倒リスクを反映しうるという可能性は、転倒という事象を予測しうるという可能性も秘めており、今後の長寿スクリーニング方法を支える上で重要なメッセージになると考える。この報告は、平成25年6月にソウルで開催された第20回国際老年学会で発表したところ、優秀ポスター演題賞を受賞した。調査フィールドの3町は、既に高齢化率が3割をこえており、ここから得られる知見は2030年以降、高齢者が3割を超える日本全体、ひいては急速に高齢化が進むアジアのデザインモデルになると考えられる。
