项目摘要

目的:超高齢化に伴う社会や医療の再構築は喫緊の課題である。その概念的フレームとして、これまでに人口転換、疾病転換、健康転換などが用いられてきたが、高齢化に伴う多様性・複雑性の増大、環境問題・リスクの表面化等により、従来の健康に捉われた概念では不十分である。そこで従来の諸概念を整理し、それらで使われた各種社会的指標を用いて、新たに「生存転換」概念として統合を試みた。方法:「生存転換」を個と種の生存技術体系の変化による生存のあり方の転換と定義し、15歳平均余命、50歳以上人口割合を指標として、歴史的変遷を分析し、転換期を同定した。更に各期に伴う個人の生涯、家族の形態、社会の構造の変遷を分析した。その転換を先進諸国や途上国、特に東アジアで検証した。定量的な解析としては、各国生命表データ・人口関連将来推計データを収集し、データベースを構築した。上記指標の動向を中心に、日本の社会構造・人口構造変化を国際的な比較のもとに、近年の東アジアの生存転換を位置づけた。社会学的分析としては、高齢化/老いを相対化してとらえるため、霊長類学からみた老い、社会史からみた老人の歴史、文化と老いに関する文献検討を行った。また、日本との比較対象として台湾の事例検討を行った。これらから高齢者の社会的統合の課題を抽出し、50歳以上を対象とする学校教育の新たな事例や第三の人生論を探索的に検討した。結果:分析の結果、50歳以上人口は、2023年に50%を超え、個人・家族・社会のレベルで、新たな考え方と制度設計が必要であることが明らかになった。特に、医療のモデルは19世紀の社会構造を前提としており、転換が求められている。医療・福祉の統合、疾病モデルの転換、提供体制の転換等の方向が判明した。今後、各課題について、より具体的な政策的研究が求められ、特に東アジアで協働していく必要があるが、そのための概念的基礎が判明した。

Results: Analysis results, population over 50 years old, 50% by 2023, individuals, families, society, new research, institutional design, necessary, clear, and timely. The social structure of the 19th century is based on the premise of special medical treatment. The direction of integration of medical care and welfare, transformation of disease management, transformation of supply system, etc. has been clearly identified. In the future, the study of specific policies for each topic will seek to clarify the basis of the concept of cooperation.
