The purpose of this study is to clear decision-making support until shift to terminal care from start of treatment by analyzing qualitatively inductively data obtained by semi-structured interview for seven Oncology Certified Nurse Specialists and five physicians.Physicians are based on evidences of treatment and mentalities of patients who want to avoid shift to terminal care,aiming an intention of improving patients'preparation and maintaining their hope.Physicians held these attitudes,explained consciously and progressively a purpose of treatment according to disease stage,a progress of a disease, and an information on palliative care.Oncology Certified Nurse Specialists based on both situations of physicians'explanation and response of patients,was going to approach with intention of preparation for decision-making of terminal care from early stage of treatment for the ultimate purpose resulting in consented among patient,family,and medical staff.

本研究的目的是通过对七位肿瘤专科护士和五位内科医生的半结构式访谈获得的数据进行定性归纳分析，明确从治疗开始到转移到临终关怀的决策支持。内科医生基于治疗证据和希望避免转移到临终关怀的患者的心理，目的是提高患者的准备和保持他们的希望。医生持有这些态度，有意识地和渐进地解释治疗目的，根据疾病阶段，疾病的进展，以及姑息治疗的信息。肿瘤学认证护士专家将根据医生的解释和患者的反应这两种情况，从治疗的早期阶段开始为最终治疗的决策做准备，最终目的是在患者、家属和医务人员之间达成一致。
