项目摘要

本年度は、意思決定における自律と最善の利益の位置づけを明らかにするために、1．意思決定の文脈に即した対応能力概念の考察、2．意思決定プロセスの検討と意思決定プロセスに寄与する具体的ツールの開発という2つの側面から研究を遂行した。具体的には、1については、前年度までの対応能力概念の考察をさらに深化させ、感情能力の位置づけに関する検討を進めた。これによって特に、意思決定に関連する情報を自身の状況に当てはめて認識し、選択肢を自身のものとして比較評価するには、感情能力が不可欠であり、一般的に評価される利益のみを目的とするのでない限り、感情能力が担う役割は対応能力概念の基礎となることを明らかにした。これは、意思決定における自律と最善の利益の均衡を明確化するための重要な観点となる。また、2に関しては、まず、意思決定ツールとしての事前指示を考察し、現在から最期に至るまでの医療ケア計画を事前に立案する一連のプロセスのなかで事前指示書の作成を適切に位置づけ、事前の話し合いを充実させる重要性を示した。次に、意思決定プロセスに寄与する具体的ツールの開発としては、前年度から継続する共同研究として、治療方針決定のための実践的フォームの作成を進めた。特に、くらしや人生設計を長期的視点から捉えつつ選択肢を比較評価する過程を含む試作版フォームをまとめ、その作成背景と内容とを関連学会大会にて口頭発表した。次年度には試作版を刊行する予定である。さらに、上記の研究を進めるにあたって、文献研究と併行して、医療ケアや死生の思想文化に関する研究会等に継続的に参加し、臨床に即した理論の構築と実践的フォームの具体化となるよう検討を重ねた。
