高齢者消化器がんの治療実態をレセプト情報ベースで調査し、実際に行われた医療行為・処方の実態を明らかにすることを目的として研究を計画した。（研究予定期間は3年）レセプトは実際施行された医療行為について医療費の請求を行うため作成されており、実際に医療の現場で行われた治療行為を反映したものといえる。一方全国のレセプト情報はビッグデータである上にレセプトの構造は複雑であり、レセプト構造およびデータ解析に精通した研究者が本研究には必須であった。我々は先行研究において全国の胃癌患者のレセプト情報解析を行った実績を有しており本研究でも医療者、医療情報の研究者さらにバイオインフォマティクスの研究者がタッグを組み研究組織を形成し研究を開始した。研究開始にあたり、厚生労働省に対して全国のレセプト情報の第三者提供の申出を行い、承認を得た（データ自体はデータ抽出に時間がかかるため現時点で未提供）。レセプト情報には医療行為や処方内容等は含まれているが患者の病態（がん患者であればがんのステージや検査結果等）は含まれていない。このため研究初年度である平成28年度は全国のレセプト情報の解析に先駆け、自施設のレセプト情報のデータベース構築および自施設の臨床データとレセプト情報の連結を開始した。さらにレセプト情報と実際の病態との間にある法則性の検討すべく、自施設のレセプト情報から薬剤の処方パターンを抽出し、処方パターンと病態との関係性の検討を開始した。今後は全国のレセプト情報を入手し、レセプト情報のデータベース構築を検討するとともに、現在進行中の自施設のデータ解析をさらに進め、レセプト情報から実際の病態を推測するアルゴリズムを構築し、そのアルゴリズムを用いて全国のレセプト情報を用いた解析を行う予定としているが、本科学研究費の研究代表者が転勤に伴い申請資格を消失したため、本科学研究費の廃止申請を行うに至った。

In addition, the regular discussion of the relationship between the information and the actual disease, the self-imposed information, the prescription and the disease, and the extraction of the prescription and the relationship between the prescription and the disease are initiated. From now on, we will start to discuss and analyze the construction of the national information system, and we will analyze the self-designed information system in progress. We will estimate the actual pathological state of the national information system, and we will analyze the national information system in advance. The scientific research fee and the research representative's application qualification disappear, and the scientific research fee and the application are terminated.
