近年の研究からHPV関連中咽頭癌は分子サブタイプで層別化されることが示されているが発癌メカニズムは不祥な点が多い。そこで本研究ではHPVゲノムの組込みによるクロマチン変化、特に転写制御異常に着目した発癌分子機構の解明と層別化、サブクラス毎の治療標的を解明することを主題とする。既に収集済みである約50症例のHPV関連中咽頭癌検体を用いた網羅的な遺伝子発現解析、エピゲノム解析を行い層別化に取り組む。検体を順次追加していき層別化の妥当性を上げていくと共に、治療標的候補について細胞株を用いた機能解析を行う。公共データでの検証を経て、個別化治療構築へ向けた分子サブタイプに対する治療標的の解明を目指す。上記の学術的「問い」を踏まえ申請者はHPV感染が引き起こす転写制御異常に基づく発癌機構の解明と新たな治療標的の提唱を目的として、HPVゲノムに近接する領域の機能解析および治療標的としての妥当性を検討していく。転写制御の解析において、ヒストンのアセチル化(H3K27ac)はユークロマチン構造と遺伝子発現の活性化状態に関連している。そこで、臨床検体を用いたChIP-seq ではH3K27acのヒストン修飾状態に着目する。本研究では、細胞株で明らかとなったintegrated HPV周囲の特徴について臨床検体を用いて検証する。100症例のHPV関連中咽頭癌の臨床検体を用いた転写制御異常の網羅的解析は渉猟しうる限り報告は認めず独自性は高い。HPV関連中咽頭癌を発癌分子メカニズムで層別化することは新たな治療標的の解明に繋がる創造性の高い研究である。
