終末期の苦痛緩和はいまだ不十分である。理由として、より状態のよい患者で実施された比較試験の結果が必ずしも終末期の患者には該当しないことが挙げられる。この課題を克服するには、リアルワールドのビッグデータを用いて信頼できるエビデンスを推定する方法論を確立する必要がある。本研究の目的は、緩和ケア対象の患者において、リアルワールドでの治療効果の推定とランダム化比較試験の結果と比較することを通じて、終末期における緩和治療の効果を検証する方法論を構築することにある。研究対象とする症状は、呼吸困難、せん妄、腹水である。2022年度は、呼吸困難に関して、モルヒネ vs. ミダゾラムのランダム化比較試験の研究計画書を作成し、倫理委員会に提出した。リアルワールドデータでは、モルヒネ vs. オキシコドンの比較、モルヒネの統一化レジメンにしたがった投与 vs. 従来の経験的投与に関して解析まで行い論文化した。モルヒネ vs. ミダゾラムの症例蓄積を行った。せん妄に関して、ハロペリドール/ベンゾジアゼピン vs. クロルプロマジン/レボメプロマジンの比較試験の実施可能性の検討を行った。リアルワールドデータでは、ハロペリドール/ベンゾジアゼピンの統一化レジメンにしたがった投与 vs. 従来の経験的投与に関して解析まで行い論文化した。ハロペリドール/ベンゾジアゼピン vs. クロルプロマジン vs.レボメプロマジンの比較の解析中である。腹水に関して、腹水単穿刺 vs. 腹水ろ過濃縮注入療法（CART）のランダム化比較試験のプロトコルコンセプトを検討した。リアルワールドデータでは、レセプトデータを用いて、腹水単穿刺 vs. 腹水ろ過濃縮注入療法（CART）の効果の比較の解析に着手した。
