2022年(令和4年)5月15日、沖縄は祖国復帰50年を迎えた。米軍統治下沖縄では、無料・低額医療費および官営医療の時期、自由診療の時期、医療保険法の時期を経ているが、この間、「国民皆保険(住民皆保険)」の原則が存しなかった。祖国復帰によって、このような原則は実現されたものの、その後、このような原則が浸食されている。このような場面を那覇市議会での議論、沖縄県国民保険審査会にかかわる裁決例、新聞報道などの資料を通じて、読み解いており、これにかかわる論稿を発行した。離島へ赴き関係者へ聞き取り調査をする予定であったが,新型コロナ感染症蔓延のために現地へ行くことができず、予定していた旅費は使われていない。一方で 離島診療を積極的に行っている本島内の医療機関の離島診療担当者への聞き取りをすることができている。今般、新型コロナウイルス感染に対する懸念から、厚生労働省は、新型コロナウイルス感染症が収束するまでの期間に限り、初診から電話やオンラインによる診療を実施できるという時限的・特例的な要件緩和を行い、コロナ禍の中で遠隔診療が進み、なかでも離島の医療がその恩恵を受けるのではないかと期待していたが、実際に関係者への聞き取りの中ではそのチャンスを十分に生かしていないようにも見える。関係者に聞き取りの中で明らかになった離島在住の糖尿病患者に対する栄養情報提供の実際と今後の課題について的を絞り研究をする予定である。
