総合病院における遺族支援を充実させることは、グリーフによる身体的・精神的危険性が高まる遺族にとって極めて重要である。本研究では高齢者専門総合病院における高齢患者の遺族における支援プログラムの開発を目的とする。看取りの状況を後方視的にレビューし、高齢者・グリーフ・遺族等に関する基本的な知識を医療者に質問し、実際の遺族調査結果を分析しプログラムを作成する。その上で、研修会を開催し普及活動を実施する。初年度にあたる2021年度は、高齢者専門総合病院における高齢患者の遺族支援プログラムの開発の最初の柱である「エンドオブライフ、看取りの状況を確認する」ことを目標とした。研究開始前1年間で死亡した患者を無作為に抽出し、約30名の看取りの様子を評価、検討する予定であったが、研究者がCOVID-19診療の責任者となる等、時間的制約が大きく診療録の後方視的レビューが十分に実施できなかった。いくつかの抽出した診療録の内容は統一されたものではなく、看取りの状況や遺族の様子を確認することに難渋した。特に精神・心理状態評価についてはカテゴリー化して検討することが望ましいと考えられた。2022年度は、上半期までに診療録の後方視的レビューを完了する予定であったが、COVID-19が収束せず研究の進捗が大幅に遅延した。2022年度下半期後半には患者の抽出は完了したものの後方視的レビューを終えておらず、作業進捗の遅れを取り戻せていない。２０２３年度はCOVID-19の影響が減ることから、遺族調査の実施を目指す。また、研究期間の延長を嘆願し、当初の予定にある遺族支援プログラムを開発し研修会の開催を2024年度中に行うこととする。
