本邦において日常生活に介護を要する高齢者 (以下，要介護高齢者) は、介護保険制度により様々なサービスを一定の自己負担額で利用することができる。その中でも通所介護サービスは、要介護高齢者の機能維持や社会参加だけでなく、介護者の負担軽減にも資することが期待されており、適用のある者に均質に提供されることが望ましい。しかしながら、胃ろう等の医療処置を受けて自宅で生活する高齢者が通所介護サービスの利用を希望する場合、他の利用者とは異なる対応が必要であることから通所介護サービスの利用に至らない可能性がある。またその実態に地域差があることも考えられるが、これらは定量的に明らかにされていない。本研究の目的は、自宅で生活する要介護高齢者の医療処置の有無により、通所介護サービスの利用状況に差異があるかどうかを明らかにすることである。本邦では「重度な要介護状態となっても住み慣れた地域で自分らしい暮らしを人生の最後まで続ける」ことを目的とした「地域包括ケアシステム」の構築が進められている。本研究で介護保険サービス提供の定量的な実態を示すことは、地域包括ケアシステム構築において重要であり、国や地方自治体における政策立案にも貢献しうるものである。本研究は、介護保険総合データベース (以下、介護DB) における介護レセプトデータと介護認定調査データを用いて実施する。厚生労働省に行った介護DBの利用申請は承諾されたが、データの提供が遅れており、2022年度中にデータを受領することができなかった。そこで2022年度は、2021年度に引き続き関連研究等の情報収集を行うとともに、介護DBの基礎的な情報についての予備調査を行い、データ提供後に実施する分析に向けた環境構築にも着手した。
