魚アレルギーは成人の新規食物アレルギー発症の原因として上位を占めている。甲殻類についても7～19歳発症の食物アレルギーの原因の第１位である。これまで、魚アレルギーの抗原としては主にパルブアルブミン、コラーゲンが、甲殻類や軟体動物ではトロポミオシンが同定されている。これらはほぼすべての魚に含有され、甲殻類・軟体動物も種間でアレルゲンの相同性を示すことがあり、魚介アレルギーの患者は厳しい食事制限を余儀なくされることも多い。しかしながら、原因の抗原によっては魚種や部位により含有量が異なり、調理法によって抗原性が低下する可能性が示されている。このため、抗原の同定が適切な食事指導のために重要である。研究代表者は長崎大学水産学部の協力のもと、2019年より魚介アレルギーの抗原解析に携わっているがイムノブロットの結果、既知のパルブアルブミンやコラーゲン以外にもバンドがみられる例を経験しており、未知のタンパクが抗原となっている可能性がある。このタンパクを同定し、臨床的な意義を検討することで、例えば加熱で変性するなどの既知の抗原と異なる性質のものであれば、本研究により患者のQOLの改善や健康障害の予防につながる可能性がある。本年度は魚介を摂取後にアレルギー症状が出現した新規患者4例について検査を実施した。この中には、一度魚を摂取後に皮疹が出現したために、魚介エキスを含めすべての魚を回避しており、食生活の幅が大きく制限されている患者も含まれた。研究計画のとおり、通常診療内で行う血液検査の際に追加で9mlほど血液を採取し、長崎大学水産学部へ血清を郵送し、血清中のIgＥを標的とした、ELISAおよびイムノブロットを行った。IgEで反応がなく明らかに臨床的および皮膚テストの結果からアレルギーが疑われる患者については、IgGについても検討を行った。
