项目摘要

歯科領域におけるユニバーサルヘルスカバレッジの普及の目的の一つに、健康格差の縮小が挙げられる。口腔の健康格差は世界的に存在し、国際比較研究によって国レベルの特性について検討することは、格差是正のための貴重な知見につながる。しかしながらこれまでは欧米からの知見が多く、アジア諸国における口腔の健康に関する国際比較研究は限られていた。そこで本研究では、世界的にみても高齢化が進んでいる国である日本とシンガポールにおいて、国際比較研究を実施した。日本老年学的評価研究（JAGES）とシンガポールで実施されたPanel on Health and Ageing of Singaporean Elderly（PHASE）の3時点のデータを分析に使用した。両調査の対象者の中で、3時点の調査に参加し、歯の本数に欠損のない、ベースライン時点で65歳以上の機能的に自立した高齢者を対象とした。要介護状態にない日本の高齢者35,717名と、シンガポールの高齢者1,032名を対象とし、教育年数による口腔の健康格差について検討した。調査の結果、両国において教育年数が少ない人ほど口腔の健康状態が悪いことが確認されたが、日本の高齢者では、シンガポールの高齢者と比べ、教育年数による口腔の健康格差が小さいことが明らかになった。その差は、無歯顎（歯のない状態）では、日本の方が0.4倍、機能的に必要とされる20本以上ない状態については0.83倍であった。背景として、シンガポールの方が、歯科医療費が高いことや砂糖摂取量が多いといった医療制度や文化的背景の違いが理由として考えられ、今後も口腔の健康格差のモニタリングとユニバーサルヘルスヘルスカバレッジの普及が重要であると考えられる。また、ユニバーサルヘルスカバレッジに関する国際共著総説なども執筆した。
