本研究は高齢者を対象に、ストレスの主観的評価に限らず、客観的評価として唾液・爪のコルチゾールを測定し、口腔状態がストレスにどのような影響を与えるかを縦断的に検討することを目的としている。このことより、高齢者の口腔の健康の維持は口腔や全身の疾患を予防するだけでなく、精神的健康の維持にも有用であり、ひいては高齢者のQOLの向上に寄与する根拠を示すことができると考えている。研究計画1年目である令和4年度は爪のコルチゾールの測定が初めてとなるため、まず爪のコルチゾールを測定した先行文献を収集し、各行程で我々の教室で測定が可能かどうかを確認しながら測定のためのプロトコルを作成した。これに沿って2年目は実際に少人数の対象者の爪と唾液のコルチゾールを測定する予定である。さらにその結果をフィードバックし、人数を増やしてコルチゾールの測定を行う予定である。また対象集団では既にK6による主観的評価としてのストレスを評価していたため、横断的なストレスと口腔状態（現在歯の状態・現在歯数・歯周組織状態・咬合状態・義歯の使用・嚥下障害・口腔衛生習慣）および全身状態（①基礎情報：性・年齢・既往歴・服薬状況・喫煙・飲酒、②栄養状態、食事の状況、③認知機能：Mini-Cog、④要介護状況・手段的日常生活動作：IADL、⑤社会経済的状況：教育歴・社会参加、⑥身体機能：身長・体重・BMI・握力・上腕周囲長・皮下脂肪厚・立ち上がり検査）との関連を検討した。その結果、主観的評価によるストレスと口腔の健康状態との間に有意な関連は認められなかった。一方全身状態では、IADLと栄養状態がK6と相関していた。この結果よりコルチゾールを測定する際に用いる問診内容の修正・追加の要否を検討する。
