项目摘要

患者が自宅で適切に経口抗がん薬を内服しているか確認できるリアルタイムアドヒアランス・モニタリングシステム（アドヒアランスシステム）の性能について検討を行った。本システムは、患者が内服薬を取り出したイベントを、電話回線を使用しモニタリングするシステムである。本システムの精度を確認するために、地下やビルの高層階で薬を取り出した時にそのイベントをモニタリングできるかを調査した。薬の取り出しからおおよそ15分間のうちにイベントをモニタリング出来た。本研究成果は2023年11月の第33回日本医療薬学会年会で発表するために演題登録を行った。2022年は経口抗がん薬のひとつであるレンバチニブについてアドヒアランスについての第22回日本医療薬学会年会で学会発表を行い海外雑誌に論文投稿した。対象は甲状腺がん、肝細胞がんでレンバチニブを内服している患者とした。評価項目はレンバチニブ服用開始後12週以内のアドヒアランス率、アドヒアランス低下要因とした。その結果、対象患者は102 例であった。レンバチニブの1～4、5～8、9～12 週の平均アドヒアランス率は各々92.7、93.3、96.6 %であった。アドヒアランス低下要因は、出血関連事象46 回、高血圧37 回が上位であった。出血関連事象によりアドヒアランスが低下した患者は全て甲状腺がん患者であった。これらのデーターは今後、アドヒアランスシステムを用いて前向き観察研究を行う際の参考データーとなる。また、カペシタビンのアドヒアランス低下理由を調査し論文投稿を行った。さらに、経口抗がん薬のアドヒアランス評価の重要性について、日本薬学会第143年会シンポジウム等で多くの薬剤師と共有を行った。
