项目摘要

本研究では、思春期・若年成人（Adolescent and young adult: 以下AYA）世代がん患者における情報のニーズを踏まえたAYA世代に特化した質問促進リスト（Question asking Prompt List: QPL）含む対話支援アプリを開発し、「アプリを用いた対話支援法により、受診の間の期間にも患者の情報ニーズ（診断、治療、妊孕性、就学、就労、パートナーとの問題、晩期障害等）が充分に整理され、最も知りたい情報に関する質問に呼応して医師の回答行動が引き出されることにより、質の高い対話と患者の満足度増加につながり、医師への信頼が向上する」という仮説を検証することを目的としている。研究①　AYA-QPL開発（2022年度）【方法】16-39歳のがんと診断を受けている患者（男性5名・女性5名）を対象に、インタビューガイドに基づき、これまで医療者に質問してきたこと、質問したかったができなかったこと等に関しインタビュー調査を行う。録音データより逐語録を作成し、二人の独立した評価者が質的内容分析（Fujimori et al., 2015）を行う。その結果とニーズ調査の結果（Okamura et al., 2021）からAYA-QPLを開発する。【結果】インタビューを行った患者背景(N=10)：女性5例、年齢中央値36歳（範囲20-38歳）、がん診断：肉腫3例、乳腺、白血病他各1例、診断からの年数中央値4.5年（範囲1-16年）。質的内容分析の結果、122の逐語切片は６カテゴリ（病状、検査、治療、仕事、妊娠・子育て・性生活、COVID-19を含む感染症）に分類された。特に、仕事、妊娠・子育て・性生活に関する質問項目は世代を反映した内容であると考えられた。
