项目摘要

ヘモグロビンA1c（HbA1c）は糖尿病患者の代表的な血糖管理指標であるが、ヘモグロビンの糖化されやすさには遺伝的な個人差があり、同じ血糖値でもHbA1cが高い人と低い人がいるため、HbA1cのみでは個人の特性を反映し得ない。研究の目的は、1型糖尿病患者コホートで長期の合併症予後調査を行い、HbA1cに加えてヘモグロビンの糖化されやすさを示す指標であるグリコアルブミンとHbA1cの比（GA/A1c比）を用いることが糖尿病の血管合併症と重症低血糖をよりよく予測するかを検証することである。また、HbA1cと血清糖化蛋白の同時測定での分布の差から求められるGlycation gapと、実測のHbA1c値と持続血糖測定で測定した平均血糖値から推定されるHbA1c値の差として求められるHemoglobin glycation indexという2つのヘモグロビンの糖化されやすさの指標との関係式を求め、GA/A1c比の有用性を示すことである。検討対象は、我が国最大の1型糖尿病患者コホートである小児インスリン治療研究会の登録症例である。1995年開始の第1から2013年開始の第4コホート分の連結されたデータは固定できたが、2018年開始の第5コホートのデータは未固定の為使用できなかった。本年度は、固定データを用い、第55回日本小児内分泌学会学術集会で「低血糖症例は、GA/HbA1c比の最も小さい第1四分位群で血糖管理が良いほど多い」、第66回日本糖尿病学会年次学術集会で「GA/ HbA1c比は長期間安定した各個人固有のglycation gapの指標である」、「持続血糖測定施行症例で、GA/HbA1c比が低値であるほど血糖変動が大きい」ことを報告した。現在論文化を進めている。北海道内発症1型糖尿病児の長期予後に関する研究に参加し、「生命予後と透析予後の改善、死因の急性糖尿病性合併症から末期腎不全への変化」を報告し、現在英文誌に投稿中である（第55回日本小児内分泌学会学術集会、第33回日本疫学会学術総会）。
