下痢、便秘に代表される排便困難は、病者の日常生活において気がかりになる症状である。便秘から下痢までを含む排便困難を、客観的指標のみでアセスメントできるツールがないという課題に対し、我々は、2週間の排便記録からフローチャートで排便パターンを分類する新しいアセスメント方法を開発した。本研究課題の目的は、このアセスメント方法を臨床に適用した場合の患者・看護師への効果を明らかにすることであり、研究方法は、①排便記録の記録・集計と排便パターンの分類を自動化するアプリケーションの開発、②効果検証に向けた測定指標・測定方略の検討、③排便記録のみを使用する排便管理との比較から、本アセスメントツールの有用性を検証する介入研究の3段階を予定している。2022年度は①排便記録の記録・集計と排便パターンの分類を自動化するアプリケーションの開発に取り組んだ。現在、本アセスメントツールは様々な視点から妥当性と信頼性が確認されているが、排便日誌の集計の煩雑さと不慣れなために起こり得る分類の誤りから、臨床現場では作業が煩雑となりアセスメントが不正確になる可能性がある。そこで、排便記録の記録・集計とフローチャートでのアセスメントを自動化（アプリケーション化）することでその問題を解決し、臨床現場での適応について検証する方針である。排便日誌アプリケーション開発委託業者を公募型プロポーザル方式で広く募集し、提出された企画提案書等の審査により1社に決定した。その後、開発担当者とのミーティングを重ね、設計、設計レビュー、デザイン作成、デザインレビューの工程を進めた。今後は、実装・テストの工程を経て2023年4月末に排便日誌アプリケーションの開発が完了する予定である。
