令和4年度は『障害者（児）支援施設の業務内容・感染管理体制の把握とリスク評価』に取り組んだ。1．クラスターを経験した障害者支援施設で事業継続のために講じた対策や困難であった対応についてインタビュー調査を行った。1）感染対策で二次的に必要となる物品（ディスポ食器、ごみ袋など）が不足した。2）法人内で職種に応じた人員確保を検討するよう厚生労働省では推奨しているが、補充した人員が感染者や濃厚接触者となり自施設に戻れなくなる可能性がある、利用者の個別性が極めて高いため直接介護は不可能であること、などから人員補充は検討しなかった。3）陽性・濃厚接触者の支援は日単位で担当制とすることはできたが、継続して担当することは困難であった。4）優先業務の検討ができていなかった。5）職員の陽性者や濃厚接触者は少なかったにもかかわらず、スタッフからは業務過多のため夜勤スタッフの増加を望む声があった。2．感染症発生時のリスクをスコアリングする「感染症発生者及び感染対策状況に基づく感染対策リスク評価」について検討した。1）アウトブレイク経験の有無は、施設としての経験ではなく個人としての経験の有無を重要視する必要がある（離職者が多い）。2）呼吸器感染症よりも感染性胃腸炎の方が業務負荷が大きい実情を踏まえ、スコアに反映させる。3）内科系の基礎疾患を有する利用者よりも、精神疾患を有する（例えば行動障害等）利用者の方が濃厚な対応を必要とすることから、基礎疾患はかなり重要な要因となる。4）医療的ケアを必要とする利用者は極めて少ないため、医療処置として項目化する。5）医療機関との連携について、施設からの一方的な情報提供に留まっている現状がある。以上のことから、今後は業務の優先度を設定し施設内人員と業務を連動させ妥当性について検討し、入所者・職種別の感染者または濃厚接触者の割合に応じたBCPの完成を目指す。
