重症ケアの場で活用される主な評価指標は,Donabedian Modelを応用したものが多い（前掲）。現状では,表1のStraructure, Process, Outcomeに即して,人工呼吸器装着日数,再挿管率,合併症の発症率などについて治療の効果として医学的な指標を臨床で用いて,患者の容態・状態を評価される場合が大半である。しかしながら,急性・重症患者の治療を行う重症ケアの場では,患者に対して集中的,かつ多角的なアプローチがなされ,生命維持と身体の回復はもちろんのこと,精神面での回復が重要視される中,看護師の果たす役割,存在意義は大きいと考える。ところが,そのアウトカムは,繰り返しとなるが人工呼吸器装着日数,再挿管率,合併症の発症率など治療の効果として評価されるのが実情である。クリティカルケア看護のアウトカムとして示される看護学的指標の創出は,今のところ皆無であり、早急の課題である。そのため,本研究課題の継続的探究により,看護ケアによる患者の前向きな変化,いわゆる回復の度合いをアウトカムとして評価できる指標が必要と考えた。当該看護実践モデルは、COVID-19の事例でも当てはまり、COVID-19や次のパンデミックの際にも重要で意義深い示唆が得られる。2022年度は、倫理申請をするために、回復を観るための項目案を分担並びに連携研究者連携研究者と共に作成した。この案をもって、倫理員会に申請をして承認を得る予定である。
