项目摘要

本研究では、腎移植レシピエントの精神面を支えアドヒアランスを高めるアプリケーション・ソフトウェア(以下app)を開発すること、appの有効性を検証しあらたな精神的介入方法を見いだすことを主な課題とし、腎移植レシピエントの移植臓器の長期生着および良好な健康状態の維持を実現することを目的としている。今年度は、三重大学医学部附属病院を研究フィールドとし、同施設に通院する同意を得られた腎移植レシピエントにappを導入し、appの活用によりアドヒアランスが向上した腎移植レシピエントの症例を検討するとともに、appによる支援内容について検討した。appによる支援内容の検討については、腎移植後フォローを行っているレシピエント57名を対象とし、お知らせ・服薬管理・健康記録機能を有する移植後管理appを使用し、app導入前、導入2ヶ月後にアンケート調査を行った。質問内容は、1. app使用継続の意向、2. 自己管理の支えになることはあるか否か、3. appに望むこととして役立つと思った機能、4. 希望するお知らせが届く頻度・内容等である。その結果、1. について、93％がappを継続利用することを希望した。2. について、app使用後には自己管理の支えがあるとした割合が有意に上昇した（63％ vs 89％；p＜0.001）。3. について、もっとも役立つ機能はお知らせ機能であった。4. について、お知らせの届く頻度は1カ月に1回程度、内容は医療情報を最も多く希望した。このような結果から、appを導入したレシピエントは、appを利用した自己管理を肯定的に捉えており、appは自己管理を支えるツールの一つとなる可能性が示唆された。またapp使用を継続させ、効果的な支援を行うには、お知らせの頻度・内容の工夫が重要であると考えられた。
