本研究は、慢性呼吸器疾患患者の病期の段階的な移行や病みの軌跡に合わせ、患者が家族、医療者とAdvance care planning(以下ACPとする)を共有し、意思決定を支えるための看護ケアモデルの構築をすることを目的としている。本年度は、慢性呼吸器疾患患者に対し、熟練看護師がどのような支援を行っているのかに着目し、インタビュー調査の分析と文献検討を中心に行った。熟練看護師へのインタビュー調査から、看護師は、患者と医療者が途切れないよう、看護師がどのような支援を行えるかを伝えながら、患者や家族の病気や治療だけではなく、療養環境、生活歴を知る対話を大事に関わっていた。それは、外来から入院、退院後と、患者と医療者が途切れない関係性を作ることを目指し、この途切れない支援を行うことで、患者の日常生活や価値観にこだわって聴き、意思決定の必要な時に価値が置かれる事柄を理解すること、また、日々の生活状況や療養への取り組み、その思いを捉えることで、変化している状況を見極めることにつなげていた。それは、意思決定をしなければならない状況やタイミングをみはからうことにもつながり、安定期と不安定期を推移する慢性疾患特有の関わりであった。また、看護師は継続的に関わることを通して患者・家族との関係性の構築だけではなく、院内外の医療チームをコーディネートする役割も果たしており、患者や家族が選択した意思決定を叶えるためのチームをつなぐ役割をもっていた。また、そのようなチームから熟練看護師が支えらえることもあり、看護ケアモデルとして構築する上でも医療チームとの協働の必要性が示唆された。
