本研究の目的は、慢性呼吸器疾患患者の診断から臨死期までのアドバンス・ケア・プランニングの過程において、リレーショナル・オートノミーに基づいた継続的な患者支援モデルを構築することである。研究1年目の令和4年度では、主要概念の明確化とインタビューガイドの作成を目標とし、研究活動を推進した。リレーショナル・オートノミーに関する文献検討を実施し、国内外における研究動向と課題を把握した。和文論文においては、2020年から2021年までの4論文が該当した。英文論文においては、1976年から2022年までの699論文が該当し、報告数は年々増加傾向にあった。そのうち選定基準を満たす108論文を対象として調査した。エンド・オブ・ライフケアにおける関係的自律の意味・活用法を検討したシステマティックレビューや、慢性疾患患者のアドバンス・ケア・プランニングへの活用を検討したもの、慢性疾患の自己管理における個人主義的自律と関係的自律との関係を検討したもの、COVID-19パンデミックにおける哲学的教訓、家族による意思決定と関係的自律の関連を検討したもの、高齢者ケア施設における関係的自律概念の活用など、その主題は多岐にわたっていた。自律性の尊重は、近年の生命倫理や意思決定支援において重要な原則となっている。しかし、英語圏においても個人主義的なアプローチへの批判的意見が聞かれるようになり、リレーショナル・オートノミーを主題とした研究が数多く行われていた。リレーショナル・オートノミーは新たな概念であり、現時点では明確な定義や実践における運用への具体的な方法が見出されていなかった。特に、国内においては研究の蓄積が乏しいことが明らかとなった。この概念を適切に使うためには、さらなる理論的・実践的な発展が必要となるという示唆を得ることができた。この研究成果を看護系学術集会にて報告した。

The new concept, the present point, the clear definition, the practical application and the concrete method of the new concept can be seen. Special, domestic research and accumulation are lacking. This concept is appropriate to the development of theory and practice. The results of this research are presented at the Academic Conference of the Department of Nursing.
