项目摘要

2022年度は、研究目標の１つ目であるバセドウ病とともに生きる女性における逆境とレジリエンスの明確化を行うために、国内外の文献レビューを行った。バセドウ病患者のレジリエンスに焦点を当てた研究は国内外でなかったため、まず国内の研究を対象に慢性疾患患者のレジリエンスに関する文献レビューを行い、2022年12月の日本看護科学学会で発表した。慢性疾患患者のレジリエンスにおける逆境と困難は、手術や術後補助化学療法、血液透析、など侵襲性の高い治療が多かったこと、がんの治療では重篤な副作用を乗り越える患者の力やその力が発揮される過程が、非がん疾患ではリハビリテーションを経て社会適応する力やその過程が着目されていることから、慢性疾患患者のレジリエンスは疾患や治療の影響をうけながら発達し、発揮される力と考えられる。抗甲状腺薬の寛解率は幅があり、再発率は１年で26％とされ、薬効が無効の場合は131I内用療法か外科治療が選択されることから、薬物治療の継続や治療選択はBD患者にとって逆境や困難になりうると考えられた。以上のことから、患者のレジリエンスでは好発年齢から就職や妊娠といったライフイベントにも着目する必要がある。次に、レジリエンスの先行要因に関連した理論で看護学、心理学領域で研究がすすでいる不確かさ（Uncertanity)に焦点をあて、海外論文の文献レビューを行い、2023年3月の国際学会で発表した。この研究では、非がん患者の不確かさを減少させるためには、睡眠の質を改善するケア、学習者中心の教育、スティグマへのケア、さらにレジリエンスのような個人のポジティブな資質に着目することが必要であることが示唆された。以上の研究結果をもとに、今後は、バセドウ病の治療やケアに関わる医療、心理の専門家を対象にした面接調査を行うための準備をすすめ研究の倫理審査を終えたところである。
