项目摘要

本研究では，口腔交互反復運動（Oral Diadochokinesis : OD）検査を発声発語器官の協調運動を評価する指標の一つとして捉え，OD検査が発達性協調運動障害（Developmental Coordination Disorder：DCD）をスクリーニングするための鑑別指標となり得るか明らかにすることを目的としている。OD検査とは，/pa/など特定の音節をできるだけ速く繰り返し構音させ，一定時間内における反復回数を測定するものである。OD検査の一般的な教示方法として，口頭指示および検査者によるデモンストレーションが行われるが，子どもを対象とした場合には，一般的な教示方法では最大能力を引き出すことが困難という指摘がある。そこで，第一に，実施方法が簡便であるOD検査の特徴を損なわず，子どもの最大能力を引き出す教示方法について検討した。対象は定型発達の年長児（5~6歳，46名）であり，教示として提示する音声モデルの速度が課題遂行に及ぼす影響を検討した。音声モデルの速度として，幼児の平均的なOD速度と，幼児の平均速度を明らかに上回る成人の平均的なOD速度の2種を設定した。分析は，言語聴覚士による聴覚的評価および音響分析を用いた。その結果，幼児を対象にしたOD検査において，幼児の能力を上回るOD速度の音声モデルの提示が，幼児の能力をより引き出すために有用であることが示された。DCDのある児では，OD検査のように，できるだけ速くという負荷条件下では，協調不全が顕在化し，反復リズムの安定性が保持できなくなる可能性が考えられる。従って，第二に，本研究で得られた子どもの能力をより引き出すことができる教示方法を用いて，学齢児の各年代のOD速度および反復リズムの安定性を明らかにし，DCDとの関連について検討する。
