本研究開発課題においては、超高齢化社会を迎えるこれからの日本社会で、課題となっている在宅医療のリソースの効率配分について、解決策を検討する。この課題解決のためには、患者情報のデジタル化が必須の要件でなる。しかし、既存の電子カルテシステムには、日常的な患者の主観情報や身体所見を、デジタル化されたデータとして収集する機能は標準仕様としては備わっていない。研究者は、臨床研究で既に広く活用されている電磁的患者報告型アウトカム（electronic Patient reported- outcome：ePRO）データ収集システム（以下、ePROシステム）に着目した。在宅医療の現場に、ePROシステムを転用し、電子カルテシステムとのデータ連携を通じて、①患者の予後改善効果、②在宅医療サービスの質の向上、③リソースの効率配分の三つについて探索する臨床研究を立案した。本研究課題申請時の研究計画においては、無作為化比較試験を実施する予定としていたが、多施設前向き観察研究として、研究デザインを変更した。都内の2つの在宅医療実施機関において、ePROシステムを用いた遠隔モニタリングが患者予後の改善に有効かどうか、また、医療者や介護者の負担が軽減されるか、について新たにヒト指針下の臨床研究として実施するよう準備し、2022年9月28日に東北大学の倫理審査委員会において研究実施計画・実施許可の承認が得られ、臨床研究の登録を完了している（UMINUMIN000049787）2023年3月までに、在宅医療実施機関においても実施許可が取得され、現在、被験者のリクルートを開始している。ePROシステムの構築については、株式会社スーザック社、株式会社アクセライト社にセットアップを委託し、今後、各実施医療機関の電子カルテとのデータ連携についても開発を進める。

This study explores the effective allocation of medical care in the home and its solutions to the problem of ultra-high-level society in Japan. This problem is solved by the necessary elements of patient information transformation. In addition, the existing electronic communication system provides daily patient information, body observations, and data collection functions. The investigators focused on the use of electronic Patient reported-outcome (ePRO) data sets in clinical studies. Clinical research was initiated on the application of ePRO system in the field of home care, communication of electronic system and data,(1) improvement of patient outcomes,(2) improvement of quality of home care system, and (3) exploration of the distribution of system effectiveness. The research plan of this research project shall be revised according to the requirements of the research project. 2. In the capital, the in-house medical practice organization, ePRO system, and remote monitoring system are used to improve the patient's future, reduce the burden on the medical practitioner and the caregiver, and prepare for the implementation of clinical research under the new guidance. The Ethics Review Committee of Tohoku University approved the implementation plan and implementation permission on September 28, 2022. Registration of clinical study completed (UMININ 000049787) March 2023, in-house medical practice license obtained, current, patient registration started The construction of ePRO system is entrusted to us by Startek Co., Ltd. and Aoktek Co., Ltd., and the development of electronic system of medical institutions is progressing in the future.
