本研究では、心房細動を有する要介護高齢者に対する抗凝固療法の有用性について検討するべく、要介護高齢者を対象に医療・介護レセプトデータを用いて後ろ向き観察研究を行う。要介護高齢者を中心とした抗凝固療法の有用性についての研究は未だなく、本研究を実施することにより、適切な指針の作成に貢献することで、患者、担当医、双方が適正な抗凝固療法を実施しえるようになることが期待できる。心房細動を有する要介護高齢者を抽出し、抗凝固薬内服の有無によって脳梗塞発症リスクに違いが出るかどうかを検討する。また、抗凝固療法以外でどのような因子（内服アドヒアランス、認知機能、寝たきりかどうか等）が脳梗塞発症リスクに関連するのかを検討し、指標として構築する。研究①：訪問診療における、心房細動を有する要介護高齢者を、カルテを用いて抽出する（300名）。心房細動の抽出方法：診療記録ならびに健康診断の心電図検査より、心房細動の有無を検討する。健康診断で心電図検査がなされていない対象者については心電計を用いて、心房細動の有無を評価する。研究②：医療・介護保険レセプトデータ（千葉県柏市）を用いて、心房細動を有する要介護高齢者を抽出する（約10000名）。評価項目：研究①：年齢、性別、BMI、血圧値、抗凝固療法の有無、抗血小板薬の有無、PT-INR、要介護度、内服管理をする介護者の有無、既往歴、転倒歴、降圧薬の有無、薬剤数（PIMの薬剤数）、CHADS2スコア、要介護認定調査票、イベント（脳梗塞、消化管出血、死亡）などを、カルテを用いて収集する。研究②：年齢、性別、抗凝固療法の有無、抗血小板薬の有無、要介護度、既往歴、薬剤数（PIMの薬剤数）、CHADS2スコア、要介護認定調査票（身体活動度や内服アドヒアランスの聴取票あり）、イベント（脳梗塞、消化管出血、死亡）などを、医療・介護保険レセプトデータから抽出する。

This study aims to investigate the usefulness of anti-coagulation therapy in patients with atrial fibrillation and in patients with atrial fibrillation. The study on the usefulness of anti-coagulation therapy for patients with hypertension has not yet been completed. The study has been carried out in a timely manner and has contributed to the development of appropriate guidelines for patients, doctors and both parties. The presence of atrial fibrillation, the presence of anticoagulants, and the absence of infarct symptoms are discussed. Factors other than coagulation therapy (internal medicine, cognitive function, sleep, etc.) are related to the development of infarction. Study ①: During visits to diagnosis and treatment, patients with high heart disease should be protected from atrial fibrillation and selected from the market (300 patients). Atrial fibrillation extraction method: diagnosis and treatment records, health diagnosis and ECG examination, atrial fibrillation presence and absence of examination. For health diagnosis, ECG examination may be used to assess the presence or absence of atrial fibrillation. Study 2: Medical and nursing care providers (Kashiwa City, Chiba) were selected from approximately 10000 patients who needed medical care. Evaluation items: Study ①: Age, gender, BMI, blood pressure, presence or absence of anti-coagulation therapy, presence or absence of anti-platelet agents, PT-INR, degree of intervention, presence or absence of mediators, history, history, presence or absence of anti-hypertension agents, number of agents (PIM agents), CHADS2 status, failure of identification of intervention, information (infarction, gastrointestinal bleeding, death), information, information on use of agents.
