本研究の目的は、癌患者におけるDVTでのACE2の関与を明らかにすることである。その結果を応用し、難治性のDVTの治療に対してACE2をターゲットとした新規治療方針を臨床展開することが期待される。従来の抗凝固療法では癌患者のDVT治療としては不十分であるので、癌患者における難治性DVTの治療に際して、ACE2をターゲットとした新規治療方針を臨床展開するための基盤を培うことを意図し、DVTにおけるACE2の関与を本研究で明らかにする予定である。症例集積ならびに研究実施計画は下記のとおりである登録目標症例数は、脱落症例も考慮し、癌に合併したDVT症例120例以上が必要であると考えている。対象は抗凝固療法の施行の有無にかかわらず癌患者のDVTとする。癌患者は次のものとする。①上皮由来の癌種だけでなく肉腫や白血病などの血液悪性腫瘍を含めた全ての悪性腫瘍を含む患者　②過去6ヶ月以内に診断された癌患者　③再発性・局所進行性あるいは転移性の癌患者、癌の治療中あるいは研究参加6ヶ月以内に癌に対する何らかの治療を受けた患者　④完全寛解にいたっていない血液悪性腫瘍患者。癌患者のDVTに対して、その診断から1週間後と4週間後に下肢静脈エコー検査を施行して初回診断時のエコー所見と比較し、血栓の進展を定量的に評価する。さらに、DVTの初回診断時ならびに1週間後と4週間後に、患者の血中ACE2ならびにAng-(1-7)濃度を計3回にわたりELISA法を用いて測定する。現在の研究の進捗状況は、患者のリクルートを目下実施中であるが、COVID-19 の流行にともない、予定より研究の施行が遅れているために、結果解析には至っていない。今後、患者のリクルートにより注力していく方針である。

The aim of this study was to investigate the relationship between ACE2 and DVT in cancer patients. The results are expected to be used in the treatment of refractory DVT. Recent advances in anti-coagulation therapy for DVT in cancer patients have been made in the treatment of refractory DVT in cancer patients. The number of cases, the number of drop-out cases, the number of cancer cases, and the number of DVT cases were considered. For example, the presence or absence of anti-coagulation therapy in cancer patients with DVT. Cancer patients have to be careful. (1) Epithelial-derived cancer types include leukemia, hematologic tumors, and all hematologic tumors.(2) Patients diagnosed with cancer within the past 6 months.(3) Patients with recurrent, locally progressive, and metastatic cancer.(4) Patients receiving treatment for cancer within 6 months.(5) Patients with hematologic tumors. The diagnosis of DVT in cancer patients was performed within 1 week and 4 weeks after diagnosis, and the findings at initial diagnosis were compared and the quantitative evaluation of thrombus progression was performed. At the time of initial diagnosis of DVT, ACE2 and Ang-(1-7) concentrations in the patient's blood were measured 1 week later and 4 weeks later.
