手術侵襲に起因して発症する術後せん妄は、医療における重要課題であり、超高齢社会に呼応し適応が拡大する高齢者の手術成績や、Quality of Life(QOL)の維持に深刻な影響を及ぼしている。またせん妄が脆弱性を示すサインではなく、認知機能低下の原因となる可能性が示され、重要性が高まっている。しかしせん妄発症の機序はほとんど分かっておらず、従って確立した予防法がないため、新規予防法開発が切望されている。国内外で免疫学的な機序に関心が集まり、末梢血の遺伝子制御解析などから発症機序の探索が始まっているが、末梢血全体の解析にとどまっており、創薬の標的となる「免疫細胞」および「免疫制御機構」を特定できていない。本研究は、Th17・Tregを介する過剰な免疫応答が術後せん妄を惹起すると仮説を立て、術後せん妄予防を目的とした二重盲検化比較試験の情報と試料を活用することで、術後せん妄発症を制御する免疫ランドスケープを解明し、発症機序に基づく術後せん妄予防法開発を進める。令和4年度は令和4年4月より二重盲検化比較試験を開始し、同時に付随研究として本研究を開始し、国立がん研究センター中央病院にて血液検体を保存した。10月には遺伝子発現解析、メタボローム解析、免疫学的解析に着手した。また令和5年2月には国立がん研究センター東病院での血液検体の保存を開始した。順調に検体保存を進めており、令和4年度は6時間以上の高侵襲手術を予定された65歳以上の103症例から前向きに血液検体を保存した。

In April 2004, a double blind comparative study was started and blood samples were preserved at the National Central Hospital. Oct. In order to preserve the blood sample, we should reserve the blood sample in 103 cases over 65 years old.
