本研究では、血液凝固第VIII因子（FVIII）およびフォン・ウィルブランド因子（VWF）の半減期に異常を示す血友病Aおよびフォン・ウィルブランド病（VWD）患者において、FVIII/VWF遺伝子解析に加え、これら5遺伝子の一塩基バリアント（SNVs）解析を行い、FVIII/VWF半減期異常に関与するクリアランスレセプターを絞り込み、さらに遺伝子組換え実験により当該レセプター発現細胞におけるFVIII/VWF取り込み様式を野生型と比較することを計画している。R4年度は、当院小児科へ通院中の血友病A患者およびvon Willebrand病（VWD）患者のうち、臨床的必要性からすでにFVIII製剤やVWF製剤のPK解析を実施している者の中から、本研究の対象となり得る患者候補を挙げた。血友病A患者において、FVIII製剤投与時の半減期が極めて長い患者1例を見出した。VWD患者においては、DDAVP負荷試験におけるVWF上昇後の半減期が短い5例のtype1 VWD症例を見出した。このうち1例は遺伝子解析によりVWF遺伝子に既知変異を認めておらず、治療目的に投与したVWF製剤の半減期も顕著に短く、抗VWF抗体も同定できない症例であり、クリアランスレセプターの関与が最も疑われる。現在、FVIII/VWFのクリアランスレセプター候補の4遺伝子（STAB2、SCARA5、CLEC4M、LRP1）とFVIII/VWFとの関連が指摘される機能不詳のTC2N遺伝子を合わせた5つの遺伝子の解析パネルを作成中であり、上記症例において同意取得のもと、随時解析を開始していく予定である。
