炎症性腸疾患（Inflammatory Bowel Disease; IBD）の中でも潰瘍性大腸炎（UC）は、原因不明の慢性大腸炎の病態である。UC患者では、内科的治療抵抗性または発癌のため、10-30%の症例で、全大腸切除術を要する。その際、炎症のない回腸から形成した嚢を肛門に吻合することで、術後は、回腸嚢が腸管内容物や便を貯留する直腸の役割を担う。しかし、最大で約50%の患者に急性回腸嚢炎が生じ、多彩な内視鏡所見を呈する。そのうち約10-15%で慢性回腸嚢炎、約10%で回腸嚢不全を呈することが知られている。また、同様の手術を要する家族性腺腫性ポリポーシス患者では、回腸嚢炎の頻度は、極めて少ないことから、回腸嚢炎はIBDの免疫病態を反映したヒトIBD発症モデルとして注目されている。特に、回腸嚢粘膜では、組織学的な大腸化が確認されていることから、小腸粘膜大腸化に伴う炎症惹起機構の存在が示唆される。一方、研究代表者は、留学先シカゴ大学で、IBD患者で生じる回腸嚢炎の多彩な内視鏡表現型に着目し、回腸嚢の予後予測を可能にする新規内視鏡分類（シカゴ分類）を、世界に先駆けて報告した。シカゴ分類では、回腸嚢体部にびまん性炎症を呈する表現型が回腸嚢切除リスクと有意に相関することが明らかとなったが、その炎症進展機構は未だ明らかとなっていない。そこで、本研究では、回腸嚢表現型ごとに、粘膜構造、上皮遺伝子発現、腸内細菌叢を評価し、多角的アプローチによる表現型形成機序の理解から、IBD病態発症機序の解明を目指している。

Inflammatory Bowel Disease (IBD) is a Disease of ulcerative colitis (UC) and chronic colitis of unknown origin. UC patients are resistant to medical treatment, cancer, and total colectomy in 10-30% of cases. During surgery, inflammation, ileal anastomosis, postoperative ileal bowel contents, and rectal dissection are important. Acute ileal inflammation occurs in up to 50% of patients and is seen by multicolored endoscopy. About 10-15% of ileum is chronic, and about 10% of ileum is incomplete The frequency of ileal inflammation and the immunopathology of IBD are the main factors affecting the development of familial adenopathy. In particular, intestinal mucosa, histology of large intestine, intestinal mucosa, large intestine, accompanied by inflammation of the existence of the mechanism, is confirmed. A representative of the study reported on a new endoscopic classification for patients with IBD who had developed ileal inflammation and had a new endoscopic classification for possible future detection of ileal inflammation. The phenotype of ileal inflammation is not clear. This study aims to clarify the mechanism of phenotype formation, mucosal structure, epithelial gene development, evaluation of intestinal bacterial flora, multiangular phenotype formation, and pathogenesis of IBD.
