本研究では膵管内乳頭粘液性腫瘍（IPMN）および膵癌に対して外科的切除が行われた症例の、主病変部及び背景膵実質における遺伝子変異プロファイルを対比することにより、IPMN患者群における膵がん発癌高リスク群の絞り込みを行うことにある。2022年度には自身が研究代表者を務めるIPMN多施設共同観察研究のデータ取集およびIPMN患者における発癌率の解析を実施し、2023年4月の日本消化器病学会総会にて報告した。解析結果としてIPMNが膵発癌の危険因子である点を明らかにした。IPMNおよび膵癌組織における背景膵に対する遺伝子変異解析については、研究代表者が名古屋大学医学部から藤田医科大学医学部へ異動したこともあり、解析症例の収集やレーザーマイクロダイセクションを実施する研究体制の準備のために時間を費やした。2022年度にはこれまで切除を実施したIPMN（良性例）、IPMN（非浸潤癌）、IPMN微小浸潤癌、IPMN由来浸潤癌、PDAC（IPMN非合併例）、PDAC（IPMN併存例）症例を各5-10例通出し、術前の画像所見との対比を実施した。実際の外科切除症例のパラフィン包埋組織からのDNA抽出、デジタルPCRを用いた遺伝子解析は2023年度より順次開始する予定である。また藤田医科大学においてIPMN経過観察症例に対するctDNAを含むバイオマーカー探索を実施するための前向き研究を計画し、IRBの承認を得て症例の集積を開始している。

In this study, intraductal breast mucinous disease (IPMN), tumor resection and surgical excision were performed. The symptoms, the main disease and the background of the disease were studied. The symptoms were higher than those of the IPMN patients.
