本研究では膵管内乳頭粘液性腫瘍（IPMN）および膵癌に対して外科的切除が行われた症例の、主病変部及び背景膵実質における遺伝子変異プロファイルを対比することにより、IPMN患者群における膵がん発癌高リスク群の絞り込みを行うことにある。2022年度には自身が研究代表者を務めるIPMN多施設共同観察研究のデータ取集およびIPMN患者における発癌率の解析を実施し、2023年4月の日本消化器病学会総会にて報告した。解析結果としてIPMNが膵発癌の危険因子である点を明らかにした。IPMNおよび膵癌組織における背景膵に対する遺伝子変異解析については、研究代表者が名古屋大学医学部から藤田医科大学医学部へ異動したこともあり、解析症例の収集やレーザーマイクロダイセクションを実施する研究体制の準備のために時間を費やした。2022年度にはこれまで切除を実施したIPMN（良性例）、IPMN（非浸潤癌）、IPMN微小浸潤癌、IPMN由来浸潤癌、PDAC（IPMN非合併例）、PDAC（IPMN併存例）症例を各5-10例通出し、術前の画像所見との対比を実施した。実際の外科切除症例のパラフィン包埋組織からのDNA抽出、デジタルPCRを用いた遺伝子解析は2023年度より順次開始する予定である。また藤田医科大学においてIPMN経過観察症例に対するctDNAを含むバイオマーカー探索を実施するための前向き研究を計画し、IRBの承認を得て症例の集積を開始している。

This study was conducted to investigate the relationship between intraductal papillary mucinous neoplasm (IPMN) and cancer related surgical resection. In 2022, the representative of IPMN was appointed as the representative of IPMN. The data collection and analysis of cancer incidence among IPMN patients were carried out. In April 2023, the Japanese Society of Digestive Diseases held a report. The analysis results show that IPMN has a high risk factor. The background of IPMN and cancer tissue analysis was discussed. The representative of the study was Nagoya University School of Medicine and Fujita Medical University School of Medicine. The time spent in preparing the research system for the analysis of cases was also discussed. In 2022, 5-10 cases of IPMN (benign cases), IPMN (non-invasive cancer), IPMN microinvasive cancer, IPMN-derived invasive cancer, PDAC (IPMN non-combined cases) and PDAC (IPMN co-existing cases) were removed, and the ratio of preoperative imaging findings was implemented. DNA extraction, PCR and DNA analysis of surgical resection cases will begin in 2023. Fujita Medical University has begun to collect cases of IPMN including ctDNA, research, and IRB recognition.
