项目摘要

食道扁平上皮癌における免疫療法の開発に伴い、腫瘍微小環境における免疫応答と排除機構の理解、腫瘍浸潤リンパ球の役割について注目されているが、好酸球に関してはその悪性腫瘍との関わりに関する解明が立ち遅れている。好酸球と癌発生に関連する少数の先行研究は頭頚部領域の扁平上皮癌に関係したものが多く、食道扁平上皮癌についても同様に有用な知見が得られることが期待でき、本研究が計画された。発癌過程における好酸球の関与について検討するため、順天堂大学医学部附属順天堂医院　食道・胃外科において2018年-2019年に食道切除術を施行された症例を対象とした。化学療法による骨髄抑制、免疫細胞への修飾を避けるため、術前化学療法を行わずに手術をした症例を抽出し、切除検体ホルマリン固定標本からTissue Micro Array(TMA)を作成した。はじめに28症例について腫瘍中央、辺縁部、正常粘膜について検体を作成し、EPX immunohistochemistry（IHC)を施行し、陽性細胞を好酸球として測定した。解析の結果、正常粘膜と比較して腫瘍部で有意に好酸球が多く存在しており、ほとんど全ての検体で脱顆粒と細胞外へのEPXの放出を伴う好酸球が見られた。このことから、好酸球の遊走が腫瘍特異的に起こっていることが示唆された。発癌早期の段階での関与を確認するため、内視鏡的粘膜切除症例の深達度pT1a症例について30例を追加し、薄切プレパラートを用いたIHCで検討した。これらを合わせた58例について、腫瘍の深達度（T3以上とT2以下）に分けて好酸球浸潤の程度を比較すると、早期群で好酸球浸潤が多く認められた。
