项目摘要

高知大学医学部附属病院入院中で、軽度から中等度の嚥下障害を有する嚥下障害患者のうち、認知機能や意識レベルの低下がない15例を対象に、頸部装着型嚥下モニターを用いて嚥下機能評価を行った。対象患者の年齢は55～86歳（平均76.8歳）、嚥下障害の原疾患は脳血管障害7例、食道癌術後3例、およびCOVID-19感染後後遺症、脊椎炎、腸間膜虚血、結腸癌術後、口蓋癌が各1例であった。全例で坐位での食事の自己摂取が可能であった。方法：以下に示すように嚥下機能および食事状況を評価した。1）摂食・嚥下の状況は藤島の摂食・嚥下グレードにより評価した。2）嚥下内視鏡検査：鼻咽腔内視鏡により、嚥下器官である咽頭・喉頭の形態や運動性、中・下咽頭の唾液貯留、声門閉鎖反射や咳反射の惹起性を確認した。次いで1回3ccの冷水を指示嚥下させて、嚥下反射の惹起性と嚥下後の咽頭クリアランスを観察した。これらの所見を兵頭らの嚥下内視鏡検査スコア評価法にしたがってスコア評価した。3）頸部装着型嚥下モニター装置（GOKURI）を装着した状態で、昼食（お茶、五分粥、みそ汁にはそれぞれとろみ剤を添加）を摂取させて総食事時間、総嚥下回数、食事中の誤嚥（むせ）回数を計測した。結果：対象患者の摂食・嚥下グレードは、6（3食経口摂取が可能だが代替栄養が必要）が2例、7（嚥下食で3食とも経口摂取可能）が13例であった。嚥下内視鏡検査は9例に実施し、合計スコアは3～7点で軽度から中等度の嚥下障害であった。頸部装着型嚥下モニターによる計測で、総食事時間が平均12.6分、総嚥下回数が56.6回、食事中の誤嚥回数が1.25回であった。嚥下内視鏡検査スコアと総嚥下時間や誤嚥回数との相関性はなかった。
