项目摘要

初年度は本研究課題の前段階として以下の仮説を基に研究を行った。咽頭期嚥下障害における嚥下後の咽頭残留は、嚥下内視鏡検査での評価において、主要な残留部位である喉頭蓋谷（VAL）と梨状陥凹（PS）の残留程度を区別し評価することが重要な知見になると予想される。2010年から2022年までの40症例を後方視的に見直し、咽頭期嚥下障害における唾液の咽頭残留と嚥下圧の関連性を検討した。兵頭スコアに加えて唾液貯留（Saliva residue：SR)スコアを新たに設定し、VALとPSの各唾液量を4段階(0-3点)に分類した。高解像度マノメトリー（HRM)のデータ不良は13例であった。計27例において検討した結果、VALのSRスコアは中咽頭最大圧とDistal contractile integral（DCI）、および中下咽頭DCIと負の相関があった。PSのSRスコアはHRM因子と相関がなかった。兵頭スコア項目④と嚥下時間は負の相関、嚥下速度は正の相関があった。このことから、嚥下内視鏡検査において、VALでの嚥下後の咽頭残留は、食道入口部の機能とは無関係に、中咽頭レベルでの咽頭収縮の弱さを示すことが明らかになった。また、嚥下内視鏡検査でのSRスコアリングは、咽頭嚥下障害における咽頭各部位での圧力差を評価する簡易的指標になる可能性があることを示した。

In the first part of the project of this study, the following basic studies have been conducted.
