项目摘要

本研究は、眼科領域のオンライン診療推進に関連する課題克服についてエビデンスを積み重ね、社会へ発信することを目標としているが、その第一段階である、オンライン診療でおこなう予定である、自己眼圧測定器の再現性・信頼性に関するエビデンスの構築をおこなった。具体的には、「アイケアHOME 手持眼圧計TA022 を用いた自己測定および他者測定による眼圧値の比較」として、雑誌「眼科」へ投稿し、受理され、論文として公表された。その要約を以下に示す。「目的」アイケアHOME 手持眼圧計（アイケアHOME）による眼圧日内変動測定中に自己測定と診療従事者による他者測定を同時に施行した。それらの結果を比較しアイケアHOME の精度や測定値の個体差等について検討する。「対象と方法」対象は健常人20 例20 眼（平均年齢40.8±8.6 歳，男性12 例・女性8 例，全例左眼）。9時から17時まで1 時間おきに計9 回の眼圧測定を行った。各回ともまず診療従事者がアイケアHOMEを用いて眼圧測定を行い直後に同じアイケアHOMEにより被験者本人が自己測定を行った。それぞれの眼圧測定値を他者測定値および自己測定値とし比較検討した。「結果」9 時から17 時までのいずれの測定時間においても他者測定値と自己測定値に有意差はなかった（9 時から順にP＝0.55，P＝0.91，P＝0.92，P＝0.14，P＝0.06，P＝0.06，P＝0.34，P＝0.10，P＝0.34）が，全時間帯において自己測定値が他者測定値に比べ低い症例、逆に高い症例、ほとんど差がない症例など個体差がみられた。測定時間の経過とともに2つの測定値の差が小さくなるなど練習効果はみられなかった。「結論」アイケアHOME による自己測定値は診療従事者による測定と有意な差はないが、その測定結果には個体差があるため事前に傾向を把握するなど注意が必要である。
