成人の部分欠損歯列に対するインプラント補綴において，長期経過後にインプラント修復物と残存歯の間に不調和が生じることがある．本年度はその病態の特徴を調査した．インプラト補綴後の残存歯列との不調和には，天然歯よりも低位となってしまう，いわゆる「インフラオクルージョン」や天然歯とインプラントの隣接間が開いてしまう，いわゆる「オープンコンタクト」が存在していた．これらの出現頻度は，それほど高く無いと考えられていたが，当院に通院してメインテナンスを受けている補綴後10年以上の患者を横断的に観察したところ，不調和の量は患者間で大きく異るも，ほとんど全ての患者に天然歯との間に不調和を認めた．特にインフラオクルージョンは，対合天然歯の挺出によって補償されて臨床上は問題が無いことも多いことが判明した．特に前歯部に生じたインフラオクルージョンと，小臼歯部に後発するオープンコンタクトは，患者のインプラント補綴後に生じる不満に繋がる事が多いことが明らかとなった．インプラント修復物と残存歯の間に不調和に対する対応としては，既存のクラウンを修正する方法と再製作する方法が取られた．オープンコンタクトは，通常インプラント近心側に生じるが，スクリューリテインであれば簡便に補綴物を外し，金属・ジルコニア冠でもダイヤモンドバーで近心部を窩洞形成しレジンを充填することで比較的簡便に修理することができた．インフラオクルージョンは再製作によって対応することがほとんどあったため，治療介入前において長期経過後に再補綴が必要になる可能性について丁寧に説明することが重要であると考えられた．
