歯科口腔領域における原因不明の慢性痛の一つに口腔内灼熱症候群（Burning Mouth Syndrome:BMS）がある。慢性痛に関する研究は腰痛を中心に近年目覚ましい発展を遂げ、感情や記憶が痛み症状に関与していることが明らかになってきていることから心理社会的介入が期待されている。しかし、口腔内の慢性痛に関してのメカニズムや治療に関する研究、特に心理社会的介入については腰痛のそれに比べて開発が格段に出遅れている。さらには、臨床現場では、患者側にBMSの症状に心理的な側面が関与していることを認めることが大きな抵抗になっているケースをしばしば認める。そこで、本研究では、BMSの患者が安心して心理社会的介入を選択できるような体制を探求するとともに、BMSに特化した心理社会的介入を開発しその治療効果の可能性について確認することを目的としている。初年度である2022年度には、愛知学院歯学部付属病院リエゾン歯科医療外来に通院しているBMSの患者に自記式アンケートを実施し、2023年3月現在、35ケース収集した。内容は次のとおりである。①口腔内の痛みの強さ、性状、②抑うつ状態・不安状態の確認、③ 痛みに対する考えや感情、④痛みに対して心理的支援を受けることへの躊躇とその理由、⑤日常生活の中で、口腔部における痛み症状と関連した生活上の困り事、あるいは、その痛み症状が増強した具体的な状況。アンケートへの協力を得られた患者の中で同意が得られた者については、生活のしにくさについてさらに詳しくインタビューを追加している。
