项目摘要

purposeIt is important to study the relationship between serum concentration of antidepressants and clinical response. In most studies total concentartion of antidepressants was determined. However the effect of antidepressant is related to free or unbound drug concentration. The purpose of current study was to clarify the relationship between free concentration of antidepressant and clinical response and to determine clinical predictors to antidepressants.methodWe determined serum concentration of clomipramine (CMI) and its metabolites, desmetylclomipramine, hydroxyclomipramine and desmethyhydroxyclomipramine by high performance liquid chromatography with UV detector.Patients included males and females who were experiencing a major depressive episode diagnosed according to DSM-III-R criteria. Patients were received CMI, 75 mg/day for three days and 150 mg/day for 4 weeks. The Hamilton Rating Scale for Depression (HRSD) was used throught the investigation.If the HIRSD at 4 weeks >=50%, a patient was classified as a responder.resultsOf 25 inpatients treated with CMI , 11 patients responded, whereas 14 patients did not respond. There were significant differences of age, gender between two groups.The difference of serum concentration between two groups, relationship between free serum concentration of CMI and clinical response , and clinical predictors are analyzing at present.
研究抗抑郁药血药浓度与临床疗效的关系具有重要意义。在大多数研究中,抗抑郁药的总浓度都是测定的。然而,抗抑郁剂的作用与游离或游离药物浓度有关。本研究的目的是阐明抗抑郁药的游离浓度与临床疗效的关系,并确定抗抑郁药的临床预测因素。方法采用高效液相色谱-紫外检测法测定氯丙咪嗪(CMI)及其代谢物去甲基氯丙咪胺、羟环丙咪嗪和去甲羟环丙咪胺的血药浓度。患者接受CMI治疗,75 mg/d,疗程3天,150 mg/d,疗程4周。采用汉密尔顿抑郁量表(HRSD),在治疗4周时HIRSD=50%为有效。结果25例接受CMI治疗的住院患者中,11例有效,14例无效。分析两组患者的血药浓度差异、CMI游离血药浓度与临床疗效的关系及临床预测因素。
