项目摘要

本研究の目的は、医療処置やケアによる「医原性の皮膚損傷」が発生しやすい新生児集中治療室に入院する児に対して、皮膚の状態をアセスメントする方法を開発することである。本研究では、新生児に負担をかけずに実施可能な評価方法の検討と、実際にその方法を用いて医原性の皮膚損傷の実態の評価、リスク因子の検討を行う。本年度は、NICU及びGCUにおいて、いくつかの非侵襲的な皮膚状態計測方法の実施可能性を検討する調査を行った。協力施設のNICU/GCUにおいて①参与観察によるテープ剥離部位の皮膚トラブルの実態調査②治療上必要があり貼付していた後の、廃棄予定のテープを用いたテープストリッピングによる皮膚状態アセスメントの実施可能性評価③超音波（エコー）機器を用いた末梢静脈カテーテル挿入・抜去後の血管とその周囲の観察の実施可能性評価④心電図モニターによる継続的なバイタルサインと点滴漏れの関係の探索を行った。延べ336回のテープ剥離の方法とその直後の症状観察を参与観察によって行った。テープの種類や貼付部位によって剥離後の症状に違いがある可能性が明らかになった。また廃棄テープをHE染色したうえで観察した結果、角質層細胞以外の成人の皮膚でテープ剥離をした際にはみられなかった細胞が見られ、新生児特有の皮膚状態のアセスメントとして利用できる可能性が明らかになった。また延べ39本の末梢静脈カテーテルの観察においては、血管が細いため難しいといわれていた新生児でも、エコーを用いて観察可能であることが明らかになった。また、成人とは異なる基準での症状の区分が必要である可能性があることも明らかになった。さらに、心電図モニターによる継続的なバイタルサインの測定によって、特に心拍数の推移と点滴漏れに関連がある可能性が示唆される事例を検討し、今後大規模データで調査をしていく意義があると考えられた。
