HIV感染症に対する医療体制が整備され，HIV陽性者の平均余命は非HIV陽性者のそれと大差ない時代となった。しかし医療現場では継続的な受診や服薬が困難で，治療意欲に乏しい患者が少なくないとの報告もある。本邦におけるHIV感染経路の約7割は男性同性間の性的接触であるが，ゲイ・バイセクシュアル男性に対する根強い社会的偏見により，彼らは感染前から自身のセクシュアルティに関する過剰なストレス（以下，マイノリティストレス）を抱え，「生きる意味を見出せない」という心理状態にある。感染後もこの心理的問題は残存もしくは悪化すると思われ，そのことがHIV陽性者の治療意欲の乏しさに関与していると臨床心理学的には推論できるが，それを実証的に明らかにした研究は見当たらない。そこで2022年度は，HIVカウンセラーの視点から，HIV感染前のマイノリティストレスがHIV感染後の心理的問題にどのように影響を及ぼしたのかという経時的変化を明らかにすることで，HIV陽性者に対する心理的援助の意義について示唆を得ることを目的とし，HIVカウンセラーへのインタビュー調査を実施した。インタビュー項目は，①HIV陽性者の治療意欲の乏しさを感じた言葉や態度について，②感染前のマイノリティストレスが，HIV陽性者の治療意欲にどのような影響を及ぼしていると考えているか，③感染前のマイノリティストレス以外に，HIV陽性者の治療意欲の乏しさに影響を与えている要因についてどのように考えているか，④HIV陽性者の心理的問題にはどのようなものがあると考えているか，などであった。現在，インタビュー調査で得られた言語データについて質的分析を行っている。

The health care system for HIV infection is in place, and the average life expectancy of HIV-positive people is significantly different from that of non-HIV-positive people. There are a lot of difficulties in receiving medical treatment at the scene, and there are a lot of reports of patients who are not interested in treatment. About 7 segments of the HIV infection pathway in this country are related to male same-sex sexual contact, social bias against males, and psychological state before infection. The psychological problems that remain after infection are not reflected in thought, and the lack of treatment intentions in HIV-positive individuals is related to the inferences of clinical psychology. In 2022, the perspective of HIV infection will be changed, the impact of HIV infection before and after HIV infection on psychological problems will be clarified, and the significance of psychological assistance for HIV positive people will be indicated.
