本研究は食事中のFODMAP（オリゴ糖、二糖類、単糖類、ポリオール）の摂取を一時的に制限するLow FODMAP dietが、小児の過敏性腸症候群（IBS）患者の症状を改善させるか、有効性の検証を目的にしている。Low FODMAP dietによる介入の前後で腸内細菌叢のプロファイル、腸管の病理および炎症性バイオマーカーの変化を副次項目として評価する予定で研究を開始した。患者はRome基準の日本語訳に基づき抽出し、食事調査と症状の変化を紙媒体で記録することとした。当初は、スマホユーザが使いやすい食品表と症状を連携させたソフトウエアの開発を予定したが、ソフトウエアの開発においていくつかの課題が生じたため、研究計画を変更した。また、COVID19のため患者の病院への受診控えの影響を受け、対象患者のリクルートが遅延していたが、Low FODMAP diet導入例において、下痢と腹痛の改善が確認される症例が一定数経験された。著効例の一部はFODMAPのうち「小麦」の中止が有効と考えられ、当該症例では好酸球性胃腸炎、セリアック病、小麦不耐症との鑑別が重要と考えらた。有効例では、内視鏡検査やtTG-IgA抗体の測定を行なった。なお、腸内細菌叢のプロファイルと炎症性バイオマーカーの解析について、共同研究を計画している。過敏性腸症候群は思春期の症例が多く、羞恥心などから便性の把握が難しい。このため便性の客観的評価として、ベッドサイドのワイヤレス超音波による直腸便性の検討を同時に進め、症状とともにLow FODMAP dietの有効性の評価を進めていく。以上の臨床研究に加えて、IBSは小児の反復性腹痛の原因として最も頻度が高く、学校生活にも支障をきたすことから、患者・家族・一般市民の本疾患への理解の促しも必要と考える。

The above clinical studies add to the research on the causes, frequency, and obstacles in school life of IBS and recurrent abdominal pain in children.
