消化器がん術前のサルコペニアに対する運動療法介入が、術後合併症の減少や予後の改善につながると考え、サルコペニアを対象とした運動介入後の術後合併症および予後に対する影響を解明し、新たに在宅リモート形式での術前運動介入の実現可能性を探る。研究①術前運動によるサルコペニア改善と術後合併症および予後の改善：サルコペニア基準である筋量、筋力、身体機能低下を満たした70歳以上の高齢食道・胃がん患者に対して術前運動および栄養介入が合併症の減少や予後改善に寄与する可能性を検討する。在宅での術前運動および栄養介入を開始、術前入院時に身体機能の評価、BIA法を用いた体成分分析装置InBodyによりskeletal muscle　index(SMI)を算出し、これらを初診時と比較する。患者へのアンケートInternet of Things(IoT)を用いて運動療法のフィードバックを行う。術後はサルコペニア改善群、サルコペニア非改善群における術後合併症の種類と頻度を比較し、短期予後についても検討する。研究②サルコペニア改善を目的にした術前運動療法として、在宅でのリモートリハビリテーションプログラムを導入する。インターネット経由のリモート形式で術前運動および栄養介入を行い、iPadあるいはスマートフォンを用い双方向対話形式でフィードバックを行う。術前患者に外来初診時から指導を開始し、在宅でリモートリハビリテーションを実践できる安全で継続可能なプログラムを導入し、高齢患者でも実施可能な忍容性および安全性を検討する。研究③サルコペニア改善が可能なサルコペニア指標の下限値および関連因子の解明：筋量、筋力、身体機能、運動習慣、サルコペニアの経時変化により群分けして測定することにより、運動療法の介入が可能なサルコペニア指標の下限値および関連因子について検討する。

To explore the possibility of preoperative exercise intervention in the form of a new type of digestive tract, and to clarify the effects of preoperative exercise intervention on postoperative complications. Study 1. To investigate the possibility of preoperative exercise and maintenance intervention for the reduction and improvement of postoperative complications: baseline muscle mass, muscle strength, and decreased body function in patients aged over 70. Preoperative exercise and nursing interventions are initiated at home, body function evaluation at preoperative admission, BIA method is used, and the skeletal muscle index(SMI) is calculated and compared at initial diagnosis. Internet of Things(IoT) for Patients The types and frequency of postoperative complications were compared between the improvement group and the non-improvement group, and the short-term follow-up was discussed. Study 2: To improve the quality of patients, preoperative exercise therapy was introduced. In the form of pre-operative exercise and maintenance intervention, iPad can be used in both directions. Preoperative patients are instructed to start at initial diagnosis, to practice at home, to conduct safety assessments, and to conduct tolerance and safety assessments for patients with high risk. Study 3: The interpretation of the lower limit of the index and the correlation factors of the possible improvement of muscle mass, muscle strength, physical function, exercise habit, group classification and determination of the time change of exercise loss, the intervention of exercise therapy and the possible evaluation of the lower limit of the index and the correlation factors.
