喫煙は疾病や死亡の最大原因であり、禁煙推進は国民の健康と福祉に寄与する最重要課題である。わが国においては、2006年より3カ月間の禁煙治療が保険診療として行われ、禁煙外来が全国的に普及した。しかしながら、ニコチン依存の強い喫煙者、また神経症・潜在的抑うつ状態にある患者は禁煙成功率が低い。さらに3カ月間の治療で禁煙に成功した者の内、4割以上が治療終了から9カ月の間に再喫煙に至っている。6カ月など長期に渡る禁煙治療により禁煙成功率が高まる可能性が海外で報告されている。しかし、わが国においては、禁煙治療の標準手順書において、3カ月間(12週間)で5回のニコチン管理指導終了後の具体的な診療指針については全く言及されていない。よって、3カ月間(12週間)を超える長期の禁煙治療のエビデンス構築と、そのエビデンスに基いた日常診療への応用が急務である。そこで本研究の目的は、禁煙を希望して禁煙外来を受診した喫煙者の中で、禁煙成功率が低い（ニコチン依存が高い、または抑うつ傾向が高い）喫煙者を対象に、長期（6カ月:24週間）禁煙治療（カウンセリング＋禁煙補助薬）の有効性を検証することである。

Smoking is the biggest cause of disease and death, and smoking ban is the most important issue for promoting national health and well-being. In March 2006, smoking ban treatment was introduced nationwide. The success rate of smoking cessation in patients with neurosis and potential depression was low. In the third month of treatment, smoking was successfully stopped, and in the fourth month or more, smoking was stopped. 6. Long-term anti-smoking treatment is highly likely to be reported overseas. After the end of the management guidance, the specific diagnosis and treatment guidelines for smoking control treatment are completed within 3 months (12 weeks). 3 months (12 weeks) to establish a long-term smoking ban treatment system and a daily treatment system. The objective of this study is to investigate the effectiveness of smoking cessation therapy (smoking cessation therapy + smoking cessation subsidy) in smokers with a low success rate (high dependence, high tendency to suppress smoking) and long-term (6 months:24 weeks).
