薬物療法の有害事象を評価するツールとして、米国 National Cancer Institute(NCI)が公表したCTCAEが広く用いられているが、CTCAEは医療者が行う評価であり、患者自身の苦痛評価と差異が生じているという報告がある。この事象に対してCTCAEの要素を含めたPRO-CTCAEが開発された。しかし、CTCAEとPRO-CTCAE双方の評価間でも差異が生じることが指摘されており、患者評価を捉える難しさが伺える。特に、外来化学療法の副作用症状の把握は治療継続に大きく影響を与える情報にもかかわらず、患者と関わる時間が短く、網羅的に患者の症状を把握することは難しい。本研究は、患者が抱える副作用状況を明確化していくために、ICTを活用し、CTCAEとPRO-CTCAEの統合を診療に反映するための外来化学療法におけるPRO-CTCAEを活用したリスクアセスメントを基盤とした看護ケアモデルの構築を目的としている。特に多様なライフサイクル途上にある乳がん患者を対象とする4年計画の研究である。1年目である2022年度は、外来化学療法におけるPRO-CTCAE活用の現状や現在の知見を把握するため、国内外の文献検討と診察、外来化学療法室にてPRO-CTCAEの有用性に関する研究を継続的に行い基礎データを作成した。例えば医療者が把握できない副作用は、呼吸困難感、動悸、不安や気分の変調といった患者の主観的な症状である。医療者が容易に把握できる副作用は、食欲不振、悪心・嘔吐、下痢、便秘など化学療法で一般的に起こりうる副作用であり、治療レジメンに特徴的な副作用である。これら基礎データを基に、外来の限られた時間で、網羅的に患者の症状を把握するために必要な項目を選定した。
